Universal Referral Form

The Universal Referral Form serves as the single point of entry into the MUSD Ecosystem of Care.

Rather than completing multiple referral forms for different services, students, families, staff, and approved community partners can submit one referral that connects individuals with the most appropriate resources.

Any student or family in Mono County may submit a referral through the system. Referrals for MUSD students are coordinated directly by the MUSD Coordination of Services Team (COST), while referrals for students attending other Mono County school districts are shared with the appropriate school site or agency for follow-up and coordination.

How the Process Works

1. Complete the Universal Referral Form.

2. Your referral is securely received by the MUSD Wellness Department.

3. Staff review the referral and determine the most appropriate supports.

4. When appropriate, referrals are coordinated with partner agencies.

5. Families are contacted if additional information or consent is needed.

6. Students receive coordinated services based on their identified needs.
